Shortly after you redeem your PADI eLearning you should receive another email asking you to fill out your digital forms.

These forms save paper and make sure there are no surprises when you show up for training, especially with the Medical Questionnaire.

Click the big blue button to fill out your forms

Sign and date the 3 forms, SignNow will guide you through it.

When you get to the signature box, type your name and chose your signature. You can also draw or upload. Any way you do it is valid, and legal.

Be sure to answer every medical question. If you miss one or miss a field it will appear in blue and you will not be able to complete the forms.

Upon completion you'll receive a confirmation email with your forms attached for your records. IF you answered YES to any of the medical questions print out your form and take it to a doctor before your first class.
